TITLE:
ANESTHESIA -ICE
Students are exposed to all aspects of general and regional anesthesia, including evaluation and management throughout the immediate perioperative period. Didactic lectures and interactive sessions dealing with physiologic, pharmacological and clinical situations are also stressed. In addition, the students will have the option of rotating in the critical care unit under the supervision of a critical care anesthesiologist.

1.  Schedules are emailed to two weeks prior to the start of your rotation.

2.  Four weeks is standard rotation length.  If signing up for 2 weeks, contact Shannon Foote (558-2426) prior to enrollment for approval or state in "comments" section on MedOneStop. 

Elective must be taken as four-week block to fulfill ICE requirement.  Cannot take two weeks and have it count toward ICE requirement.

Miller and Pardo:  Basics of Anesthesia, 6th edition will be provided at the beginning of the rotation.

The student will be able to:

Evaluate and manage airway, breathing and circulation of unconscious patients.

Demonstrate manual dexterity in the technical aspects of venous access and intubation.

Discuss the concepts of advanced cardiopulmonary resuscitation.

Demonstrate appropriate skills in patient preoperative management and preparation for anesthesia.

Perform all aspects of intraoperatively management of the patient undergoing surgery, including, drug administration, fluid management, and intraoperative evaluation and management of emergency situations.

Discuss knowledge of cardiac and pulmonary physiology as it pertains to general and regional anesthesia.

Demonstrate knowledge of pharmacology, including drugs utilized in anesthesia and in "acute care medicine."

Discuss cost/benefit of currently used anesthesia drugs and risk/benefit issues related to technique and agent choices.

Review and discuss outpatient vs. inpatient pharmacology and optimal patient care.

Discuss ethical and medical issues pertaining to drug addiction in relation to patient anesthesia selection and administration.

Students will have a minimum of forty (40) patient clinical encounters in their rotation and meet the established standards for completion of the Minimum Competency Form. Students will be required to submit a written case summary.

The student will be able to discuss and perform the following:

Airway management including endotracheal tube placement, LMA placement, mask ventilation and oral and nasal airway placement.

Use anesthetic vaporizers to produce inhalation anesthesia.

Select and administer intravenous (IV) agents including induction agents (to produce patient unconsciousness), maintenance agents (to maintain patient unconsciousness), opioids and analgesics (to maintain appropriate analgesia), and vasoactive agents (to maintain homeostasis).

Select and administer intravenous fluids including crystalloids, colloids and blood products.

Place and use monitors including electrocardiogram, pulse oximeter, capnography and EEG.

Appropriately choose and use muscle relaxants including competitive and depolarizing agents.

Select and administer postoperative pain control, including PCA (patient controlled analgesia), epidural and infusions.

Choose and administer agents for postoperative nausea management.

Discuss and choose appropriate regional anesthesia including Bier, spinal, epidural and axillary blocks.

SCHEDULE NOTE:

FYI:  Shannon Foote will email schedule two weeks prior to the beginning of your rotation.  Please come to MSB 3504 the week before your rotation begins to receive your course materials.  The forms you will receive are necessary for your rotation.  If you do not fill out the required forms, you will not be given a grade for your rotation.  Also, please call the hospital you are assigned to the Friday before your rotation begins to make sure all paperwork has been received and they are prepared for your rotation to begin. 

Please note:  If you are assigned to CCHMC or St. Elizabeth, please read the instructions attached to your schedule regarding your rotation as they have specific instructions.

ATTENDANCE AND ABSENCE POLICY

 

Session Attendance for M4 Students

  • Students may miss no more than two days of planned excused absences on a four week rotation without being required to make-up the work, at the discretion of the clerkship/elective/course director or his/her designee.
  • Non-AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the clerkship director to best align with the site schedule. Students may request to schedule 1 or more of these 4 days for planned absences that fall under 1 of the categories listed below for excused absences during non-AI rotations, in consultation with the course/elective director, who may or may not approve such planned absences.
  • AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the course director to best align with the site schedule. Students may request to schedule 1 or 2 of these days for planned absences that fall under 1 of the categories listed below for excused absences during AI rotations, in consultation with the course director, who may or may not approve such planned absences. Students must avoid scheduling Step 2 examinations during an Acting Internship.
  • Excused Absences - The following will be considered excused absences:
    • Diagnostic, preventative, and therapeutic health services (e.g. doctor appointments, physical therapy, counselling, etc).
    • Personal illness, accident or a major catastrophic event
    • Death or serious illness of immediate family members. Immediate family members, as defined by UC, are Grandparents, Brother, Sister, Brother-in law, Sister-in-law, Daughter-in-law, Son-in-law, Father, Mother, Mother-in-law, Father-in-law, Step-sister, Step-brother, Step-mother, Step-father, Spouse or domestic partner, Child, Grandchild, legal Guardian or other person who stands in place of parent (in Loco Parentis)
  • Whenever possible, planned absences should be requested a minimum of six weeks in advance of the start of the clerkship/elective/course in which the absence will occur; this enables the clerkship/course/elective to help plan for educational event scheduling (e.g. a known appointment could be scheduled around with enough notice and the student might not have any required coursework to make up). Absences requested less than 1 week prior to the planned absence may not be considered for a possible excused absence unless extenuating circumstances prevented the student from providing timely notification per the policy. Students should first submit their request for a planned absence to the clerkship/elective/course director using the online MSSF. All planned/excused absences for any reason should be documented on the MSSF.
  • The COM abides by the UC Religious Observance Policy that respects the religious diversity of its students by providing opportunities, where possible, for accommodation in cases where conflicts exist between students’ religious beliefs/practices and educational activities. In clinical settings, such accommodations must honor the primacy of a commitment to patient care and avoid unduly burdening faculty, staff and the general student population involved in the affected educational and/or patient care activity.
