PEDIATRIC EMERGENCY MED OP -ICE
This elective is designed for students who desire an intense experience in the field of Pediatric Emergency Medicine (PEM). The rotation involves direct patient care in all areas of the Children's Hospital Emergency Department and several conferences. The experience will be individually designed to develop the student's skills in physical diagnosis, differential diagnosis and management of acutely ill and injured children. The student will see patients in the ED and then will be precepted by a PEM Fellow and attending. He/she is expected to attend the resident PEM conferences which occur generally on a Tuesday and a Friday morning.

REPORT 1ST DAY:
The medical student will receive an email prior to the rotation requesting "must have " schedule changes and orienting the medical student to what needs to be done prior to the rotation. It is in the best interest for the medical student to respond to this email so that their schedule requests can be considered in the development of the clinical schedule. The medical student will receive a schedule sometime the week prior to the start day. The medical student will need to do EPIC training on your start day, which will be scheduled by Emergency Medicine. After EPIC training the medical student should report to the Emergency Department front desk in the new CCB (Critical Care Building) also called Building G on L2 (floor) to get oriented to the Emergency Department. The Emergency Services Representative (ESR) will let you into the clinical area. You need to make sure you get your badge prior to starting the rotation. Instructional Methods:

The bulk of learning/teaching will occur in the Emergency Department in the clinical setting. The medical student will see patients, perform histories and physical exams, present to the fellow/attending, discuss the differential diagnosis, develop a plan, and implement that plan. The fellow/attending will guide the medical student with bedside teaching. Online resources will be used as necessary, aiding in discussing latest evidence or guidelines. The medical student is expected to attend the resident lectures, medical video review, and simulations sessions in which core topics within Pediatric Emergency Medicine will be covered. The medical student is expected to participate in self-driven, independent learning about topics, disease processes, procedures that were encountered (and those not encountered) during his/her clinical shift. 
Knowledge/Skills:
  1. Develop competency in history and physical examinations in children with emergent or urgent complaints.

  2. Develop competency on indications for and interpretations of laboratory or radiographic studies.

  3. Evaluate and treat acutely ill or injured children in the Emergency Department with developing competency in medication usage.

  4. Develop skills in the use of consultation in emergency medicine

  5. Develop an understanding of the role of emergency medicine in the field of Pediatrics.

  6. Develop special knowledge regarding a topic of interest in pediatric emergency medicine.

Procedures:
  1. Complete sepsis work-up including lumbar puncture, as available

  2. Perform simple wound closures and abscess drainage

  3. Understand the indications and when possible perform:  NG placement, urinary catheterization

  4. Perform splint placement

  5. Observe resuscitations, as available

  6. Observe intubation/CPR/defibrillations, as available

SAMPLE WEEK

SCHEDULE NOTE:

 This elective is designed for students who desire an intense experience in the field of Pediatric Emergency Medicine.  The clinical time will be divided between the areas of clinical care in the Emergency Department at Children's Hospital Medical Center.  The experience will be individually designed to develop the student's skills in physical diagnosis, differential diagnosis and management of acutely ill and injured children. All clinical care will be supervised by the Division of Emergency Medicine attending and fellows. Approximately 70% of the evaluation will be on clinical grounds, and 30% attitude/professionalism. The student will work a combination of 3 shifts: 0800-1600;1200-2000;1600-2400 7 days a week.

Conferences, as part of the program, include those listed below.  The medical student is free to go to all of them.  Each monthly schedule will note if there are any changes.  In addition, the regular noon pediatric resident conferences may be of interest

08:15-11:00 Resident lecture once a month

08:30-10:30 Resident Simulation once a month (on hold during pandemic due to inability to distance safely)

10:00-11:00 Medical video review once a month

Fellow conferences--schedule varies, will be provided monthly

The medical student is expected to submit one write-up of a patient seen in the Emergency Department. The write should contain history, physical exam, differential, plan, results, disposition. It should also include a reflection section: 1) what you learned from this patient (i.e., disease presentation, differential, thought process in deciding plan, psychosocial components (delivering bad news, calming a distraught parent)); (2) what you contributed to the interaction/patient visit.

 
 

ATTENDANCE AND ABSENCE POLICY

 

Session Attendance for M4 Students

  • Students may miss no more than two days of planned excused absences on a four week rotation without being required to make-up the work, at the discretion of the clerkship/elective/course director or his/her designee.
  • Non-AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the clerkship director to best align with the site schedule. Students may request to schedule 1 or more of these 4 days for planned absences that fall under 1 of the categories listed below for excused absences during non-AI rotations, in consultation with the course/elective director, who may or may not approve such planned absences.
  • AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the course director to best align with the site schedule. Students may request to schedule 1 or 2 of these days for planned absences that fall under 1 of the categories listed below for excused absences during AI rotations, in consultation with the course director, who may or may not approve such planned absences. Students must avoid scheduling Step 2 examinations during an Acting Internship.
  • Excused Absences - The following will be considered excused absences:
    • Diagnostic, preventative, and therapeutic health services (e.g. doctor appointments, physical therapy, counselling, etc).
    • Personal illness, accident or a major catastrophic event
    • Death or serious illness of immediate family members. Immediate family members, as defined by UC, are Grandparents, Brother, Sister, Brother-in law, Sister-in-law, Daughter-in-law, Son-in-law, Father, Mother, Mother-in-law, Father-in-law, Step-sister, Step-brother, Step-mother, Step-father, Spouse or domestic partner, Child, Grandchild, legal Guardian or other person who stands in place of parent (in Loco Parentis)
  • Whenever possible, planned absences should be requested a minimum of six weeks in advance of the start of the clerkship/elective/course in which the absence will occur; this enables the clerkship/course/elective to help plan for educational event scheduling (e.g. a known appointment could be scheduled around with enough notice and the student might not have any required coursework to make up). Absences requested less than 1 week prior to the planned absence may not be considered for a possible excused absence unless extenuating circumstances prevented the student from providing timely notification per the policy. Students should first submit their request for a planned absence to the clerkship/elective/course director using the online MSSF. All planned/excused absences for any reason should be documented on the MSSF.
  • The COM abides by the UC Religious Observance Policy that respects the religious diversity of its students by providing opportunities, where possible, for accommodation in cases where conflicts exist between students’ religious beliefs/practices and educational activities. In clinical settings, such accommodations must honor the primacy of a commitment to patient care and avoid unduly burdening faculty, staff and the general student population involved in the affected educational and/or patient care activity.
