TITLE:
AMB PED PRECEPTSHP OP -ICE
This elective immerses students in the practice of primary care pediatrics. Students have an opportunity to learn about growth, development and nutrition in pediatrics with wellness visits and about common acute illness in children in focused ill visits.
PREREQUISITES:

 All assignments are generally in private pediatric practices in the community but may be placed at times in the CCHMC system. You will need reliable car transportation to get to the practice sites. Practices available are urban, suburban, and rural. If you want to rotate at a particular practice, you should contact Ms. Pence at least 8 weeks in advance. If the elective is full or closed, you can contact Dr. Lehmann. Frequently a second student slot can be opened if we have preceptor availability.

WORKING HOURS:
Usually 8:00-5:00 PM Monday-Friday, but could include nights and weekends. No overnight duties.

This course is entirely clinical. No lectures or other educational methods are used.

Knowledge/Skills:
1. Take on primary responsibility for patient care.
2. Collect histories and physical exams, and provide oral and written communication appropriately.
3. Synthesize clinical information to make common diagnoses in childhood illness (i.e. streptococal pharyngitis, asthma exacerbations, etc) and propose treatment plans.
4. Share information effectively with a patient and family, including routine anticipatory guidance for all childhood stages.
5. Identify children with abnormal growth parameters and be able to plot or recognize those parameters on a growth chart.
6. Order appropriate immunizations.
7. Prioritize and organize work effectively.
8. Demonstrate effective use of electronic or other medical record systems.
9. Function as a "team player" with faculty, nurses, ancillary staff and all others involved in the care of the patient.
10. Cope with uncertainty in patient care issues (i.e. knowing what you know and what you don’t know, accessing best resources, using evidence-based medicine, and knowing when and how to get help).
11. Coordinate the care of your patient with appropriate referrals to specialists and outside community agencies.
12. Identify barriers to care in patients, such as financial, transportation, medical adherence, cultural, etc.
13. Discuss cost effective strategies in patient care.
14. Recognize mental health issues in pediatric patients and their families and begin the treatment process.
15. Identify the impact of chronic illness on a child's life.
16. Increase knowledge base of breastfeeding medicine.

Procedures:

  1. Perform and/or assist in vision and hearing testing.
  2. Obtain throat swab specimens.
  3. Administer immunizations and other injections.
  4. Perform pneumatic otoscopy.
  5. Obtain and plot accurate weight, length/height, and head circumference measurements of children.
  6. Calculate BMI and/or recognize BMI percentiles and compare to known charts to assess overweight and obese children.
Remediation Plan:
 Aquifer modules and other Aquifer activities can be offered to replace any lost time due to illness, etc.

ATTENDANCE AND ABSENCE POLICY

 

Session Attendance for M4 Students

  • Students may miss no more than two days of planned excused absences on a four week rotation without being required to make-up the work, at the discretion of the clerkship/elective/course director or his/her designee.
  • Non-AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the clerkship director to best align with the site schedule. Students may request to schedule 1 or more of these 4 days for planned absences that fall under 1 of the categories listed below for excused absences during non-AI rotations, in consultation with the course/elective director, who may or may not approve such planned absences.
  • AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the course director to best align with the site schedule. Students may request to schedule 1 or 2 of these days for planned absences that fall under 1 of the categories listed below for excused absences during AI rotations, in consultation with the course director, who may or may not approve such planned absences. Students must avoid scheduling Step 2 examinations during an Acting Internship.
  • Excused Absences - The following will be considered excused absences:
    • Diagnostic, preventative, and therapeutic health services (e.g. doctor appointments, physical therapy, counselling, etc).
    • Personal illness, accident or a major catastrophic event
    • Death or serious illness of immediate family members. Immediate family members, as defined by UC, are Grandparents, Brother, Sister, Brother-in law, Sister-in-law, Daughter-in-law, Son-in-law, Father, Mother, Mother-in-law, Father-in-law, Step-sister, Step-brother, Step-mother, Step-father, Spouse or domestic partner, Child, Grandchild, legal Guardian or other person who stands in place of parent (in Loco Parentis)
  • Whenever possible, planned absences should be requested a minimum of six weeks in advance of the start of the clerkship/elective/course in which the absence will occur; this enables the clerkship/course/elective to help plan for educational event scheduling (e.g. a known appointment could be scheduled around with enough notice and the student might not have any required coursework to make up). Absences requested less than 1 week prior to the planned absence may not be considered for a possible excused absence unless extenuating circumstances prevented the student from providing timely notification per the policy. Students should first submit their request for a planned absence to the clerkship/elective/course director using the online MSSF. All planned/excused absences for any reason should be documented on the MSSF.
  • The COM abides by the UC Religious Observance Policy that respects the religious diversity of its students by providing opportunities, where possible, for accommodation in cases where conflicts exist between students’ religious beliefs/practices and educational activities. In clinical settings, such accommodations must honor the primacy of a commitment to patient care and avoid unduly burdening faculty, staff and the general student population involved in the affected educational and/or patient care activity.
