TITLE:
Clinical Ultrasound in Emergency Medicine
Point-of-care ultrasound is an essential tool in the care of the undifferentiated emergency department patient. In this course, students will learn how to acquire and interpret standard images for all ACEP core ultrasound applications. This is a hands-on course during which students will complete 20-hours of supervised bedside scanning in the ED in addition to various workshops on live ultrasound models and procedural task trainers. Students will engage in a range of supplemental learning modalities including independent study, didactic, and formal case discussion.
PREREQUISITES:
Enrollment will be limited to only those students who are enrolled in the 1 month AI in Emergency Medicine.  Students interested in taking the elective must submit a letter of interest to the course director or the course coordinator, Paula Klepper, of no more than 1 page in length detailing their reasons for interest in the elective and an idea for an academic project they would complete during the course. This letter would need to be submitted for review no less than 1 month prior to the start date of the planned elective. Decisions to accept students to the elective will be based on course directors review of this letter, course director may elect to have additional communications with the student to clarify plans for the elective.

WORKING HOURS:
40 hr/wk total: didactics 0900-1300, scanning in 4-6hr blocks during afternoons
REPORT 1ST DAY:
You will receive an email from the Course Administrator (Ms. Paula Klepper) prior to your first day with your schedule for the 2-week Elective. Classes will be held in the University of Cincinnati Department of Emergency Medicine Academic Offices (MSB Suite 1654).

ASSESSMENT:
 Students will be evaluated by preceptors based on their perfomance of ultrasound image aquisition and interpretation in real time during scanning shifts in the ED as well as during scheduled image review sessions. Students will complete a presentation on an ultrasound related topic of their choosing which will be evaluated by the course director.

OBJECTIVES

Knowledge/Skills:
1.  Understand principles of ultrasound safety and ALARA (keeping exposure "as low as reasonably achievable").
2.  Obtain standard images for all ACEP core ultrasound applications (Cardiac, eFAST, Aorta, Renal, Biliary/RUQ, Early Pregnancy, Soft Tissue, Procedural Guidance). 
3.  Discuss common clinical indications for all ACEP core ultrasound applications.
4.  Demonstrate knowledge of normal anatomy and pathology commonly encountered within the scope of the ACEP core ultrasound applications.
5.  Complete 20 hours of hands-on ultrasound scanning shifts with ultrasound faculty/fellows, Sonographer Educator, EM faculty, and EM senior residents.
6.  Critique ultrasound studies for adequacy, image quality, and accuracy.
7.  Gain exposure to current ultrasound research and emerging trends.

Procedures:
Students will perform each of the following studies on a live patient, a live model, or task trainer:
1.  Cardiac/transthoracic echo
2.  Extended FAST
3.  Aorta
4.  Renal
5.  Biliary/right upper quadrant
6.  Early pregnancy
7.  Soft tissue

Students will perform each of the following ultrasound guided procedures on a live patient or task trainer:
1.  Ultrasound guided peripheral venous access
2.  Ultrasound guided central venous access
3.  Ultrasound guided incision and drainage
Remediation Plan:
 Due to the short duration of the elective time and limited instructor availability, make up plan for any missed days or assignements will need to be discussed and arranged with the course director by the student as soon as possible.

SCHEDULE NOTE:
Each student will complete a total of 20 hours of hands-on bedside scanning in the ED. Students will be supervised by ultrasound trained faculty/fellows, our Sonographer Educator, or senior residents. Shifts will fall between the hours of 1100-2300 and may be variable by supervisor. 

Recurring sessions:
-  Monday: Ultrasound Fellows Didactic, Journal Club, and Quality Assurance every week from 1000-1300
-  Wednesday: EM Grand Rounds every week from 0800-1300

ATTENDANCE AND ABSENCE POLICY

 

Session Attendance for M4 Students

  • Students may miss no more than two days of planned excused absences on a four week rotation without being required to make-up the work, at the discretion of the clerkship/elective/course director or his/her designee.
  • Non-AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the clerkship director to best align with the site schedule. Students may request to schedule 1 or more of these 4 days for planned absences that fall under 1 of the categories listed below for excused absences during non-AI rotations, in consultation with the course/elective director, who may or may not approve such planned absences.
  • AI Rotations - Per the Student Duty Hours Policy, an average of one day (24 hours) in every seven must be free of clinical responsibilities (including seminars, clinic, rounds, lectures) averaged over a four week period. These days off are assigned by the course director to best align with the site schedule. Students may request to schedule 1 or 2 of these days for planned absences that fall under 1 of the categories listed below for excused absences during AI rotations, in consultation with the course director, who may or may not approve such planned absences. Students must avoid scheduling Step 2 examinations during an Acting Internship.
  • Excused Absences - The following will be considered excused absences:
    • Diagnostic, preventative, and therapeutic health services (e.g. doctor appointments, physical therapy, counselling, etc).
    • Personal illness, accident or a major catastrophic event
    • Death or serious illness of immediate family members. Immediate family members, as defined by UC, are Grandparents, Brother, Sister, Brother-in law, Sister-in-law, Daughter-in-law, Son-in-law, Father, Mother, Mother-in-law, Father-in-law, Step-sister, Step-brother, Step-mother, Step-father, Spouse or domestic partner, Child, Grandchild, legal Guardian or other person who stands in place of parent (in Loco Parentis)
  • Whenever possible, planned absences should be requested a minimum of six weeks in advance of the start of the clerkship/elective/course in which the absence will occur; this enables the clerkship/course/elective to help plan for educational event scheduling (e.g. a known appointment could be scheduled around with enough notice and the student might not have any required coursework to make up). Absences requested less than 1 week prior to the planned absence may not be considered for a possible excused absence unless extenuating circumstances prevented the student from providing timely notification per the policy. Students should first submit their request for a planned absence to the clerkship/elective/course director using the online MSSF. All planned/excused absences for any reason should be documented on the MSSF.
  • The COM abides by the UC Religious Observance Policy that respects the religious diversity of its students by providing opportunities, where possible, for accommodation in cases where conflicts exist between students’ religious beliefs/practices and educational activities. In clinical settings, such accommodations must honor the primacy of a commitment to patient care and avoid unduly burdening faculty, staff and the general student population involved in the affected educational and/or patient care activity.
